Welcome to Jean Hailes survey

This survey was developed by Jean Hailes for Women’s Health and a team of experts. It has three parts, and it takes about 10 minutes to answer the questions. The first part asks about your health and health care since the beginning of the COVID-19 pandemic in 2020. The second asks about what would help you stay healthy and what health information you would like more of. The third asks questions about you.

Answering the questions is voluntary and you can stop at any time. Your answers are anonymous, we won’t know who you are.

To thank you for your time, you can go into the draw for one of ten $100 EFTPOS gift cards if you complete the survey. At the end of the questions you can enter the draw.
